5. What is the contact person's phone number?
6. What is the contact person's e-mail?
7. When was the group founded, or when did the solo artist start performing?
8. Has the artist previously received a visa to perform in the US? Yes No
If yes, what type of visa and what was the INS/USCIS reference number? (It starts with three letters, probably EAC or SRC or LIN.)
9. What is the EXACT date of arrival in the USA for the artist?
If unsure, put the earliest possible arrival date.
